Breast Talk Mission

Our mission in BREAST TALK is to provide you with an understanding of breast issues pertinent to you personally which can empower you to make choices to either prevent breast cancer, detect it early, diagnose it early, obtain the proper treatment, decrease the risk of dying from breast cancer or having unnecessary or disfiguring surgery because of this disorder.

As breast cancer across the world represents 22% of all cancers that are diagnosed and is therefore the most common non-skin cancer and the incidence is increasing in the United States and developing countries, this is an area of concern that is crucial not only at a societal level but at a personal level.  This site is designed to meet your needs personally.

If you ask yourself what is your most valuable possession, you may reach the same conclusion I have, and that is, time in good health.  Our goal is to try to assist you in achieving time in good health that you may pursue your life, your liberty and engage in your own pursuit of happiness for your personal satisfaction and the joy of your family.  Worry about breast cancer and the proper management of breast conditions is of crucial importance to most women and there may come a time in each woman’s life when the priority is to find the proper information as it applies to her and utilize it for her own health.  That is where BREAST TALK is relevant.  Across the United States now, due to the social conditions in which medicine is practiced, there is often inadequate time by an individual physician in the clinical setting to sit down with the patient and given them the 30 or 45 minutes of uninterrupted discussion that they need in order to achieve a complete understanding of what their particular breast condition may be, i.e., fibrocystic disorder, precancerous condition, or cancerous condition and what their options are for treatment or management.  Ultimately, BREAST ISSUES ARE UNIQUE TO EACH INDIVIDUAL and breast care is A BOUTIQUE PART OF MEDICINE where EACH SOLUTION IS A DESIGNER SOLUTION PERTINENT TO THE INDIVIDUAL PERSON who is in front of his/her physician.


Need For Knowledge

BREAST TALK works when you have a need for knowledge or information and you recognize that it takes more than three to five minutes of an expert’s time to let you understand exactly what your condition is, all the nuances associated with that, and convey to you the options which you have the right to exercise in your situation. 

The traditional medical model involves a history, physical examination, ordering and reviewing laboratory results, all of which lead to a diagnosis or impression or conclusion, and then a recommendation regarding further tests and/or action, i.e., medication that needs to be taken, surgery that needs to be done, physical therapy that may be ordered, etc.  This traditional model is an invaluable tool when you have found the right healthcare provider for you in your region, or you can travel to the person you know is the expert and whom you can trust.

We are asked frequently to sit down and spend more time with a woman or man to discuss her/his own particular condition.  The conditions we get asked to discuss are the genetics of breast cancer including BRCA testing, what does a particular imaging report mean and what are the implications of it and what should be done as a consequence of it, i.e., abnormal mammogram, ultrasound, or MRI report.  In many cases, we need to visit with women regarding an abnormal pathology report, explain it to them, put it in the perspective of the big picture, and focus down on what it means for them.


Personalized Discussion

BREAST TALK represents an opportunity for you to engage an expert in the field of breast disease and breast health, to have up a 50 minute personalized discussion with you done over your webcam or over the telephone, whichever is appropriate for you. 

We ask in advance as you engage our service, that you fax us copies of any pertinent breast pathology reports or reports regarding mammograms, ultrasounds, or MRI’s.  When you visit with us, it is for the purpose of meeting your needs.  In general, we would like to structure this by asking you to tell us what you expect to get out of this session as we open the session.  Based on the vast knowledge and experience, we will do our best to meet your needs.  We will explore the issues you present and raise some issues that should be raised that you may be unaware of.  We will provide you with information which may lead you to new conclusions about actions you may wish to take.  For example, you may tell us that you have a thickening in the breast and you had a mammogram which shows nothing and you want to explore that issue further.  We will answer your questions, which will give you the guidance you need to pursue the rest of your workup for detection, diagnosis, and treatment.  In your case, you may have been diagnosed with a breast cancer or premalignant condition, but wish to achieve a more profound understanding of that particular condition as it relates to you in order for you to have options regarding choices of therapy, choices of therapists, and location of therapy.  That is where BREAST TALK can play a role because we can, in an interactive way, in a generous amount of time provided, explore with you the issues to allow you to reach your own conclusions.


Course Options

At the end of your BREAST TALK session, we expect to have had a dialogue with you germane to your particular case, after which you may choose to take any one of the courses:

1) You may go back to your original healthcare provider with request for certain other types of imaging or views or studies.  You may go back and have discussions regarding a different form of therapy knowing the proper questions to ask regarding this and issues to be resolved in advance of such therapy.

2) You may choose to visit with a breast surgeon or medical oncologist in your area different from anyone else that you may have visited with before.

3) You may choose to come to Palm Beach County directly so that we can do a true formal consultation for you involving the paradigm as above, that is to say, do a proper examination, direct review of laboratory data, etc.  What we are not going to do is practice medicine over the telephone or the internet as defined in the traditional medical model above.  What we are not going to do is violate your privacy by giving your information to anyone else other than ourselves or the consultants selected by the directors of this organization for the purpose of engaging in a dialogue with you.


Services and Cost

We have four divisions of service that we can provide to you:

Charges for Services

Service 1

The first is a nurse practitioner BREAST TALK, which is specifically dedicated to breast cancer risk analysis and discussion of genetic breast cancer and the testing and all of the information that pertains to that as it relates directly to you.*

$350.00

Service 2

The second will be a breast healthcare practitioner as a general surgeon who is a specialist in breasts, or a breast surgeon who devotes his or her time completely to breast disorders and is conversant with the literature and is actively engaged in the practice of breast cancer detection, diagnosis and treatment at the present time.*

$495.00

Service 3

The third level of service would be a situation where your personal story can best be talked about between yourself and a medical oncologist who is a team member of BREAST TALK.  The above three situations will be ones where you fax or email to us the information, which may include your own or a professionally written summary of your condition at this point, and then copies of any mammograms, ultrasounds, MRI, pathology or cytology reports.  We will review these ahead of time so by the time we are in front of you by telephone or video conferencing techniques, we will be somewhat conversant with your situation and can make best use of the time allocated.*

$495.00

*These services will be scheduled and completed within the week (seven working days) in which you request them and engage us. 

Service 4

Service 4 is a more comprehensive review in which you may fax us or FedEx to us mammography films, ultrasound films, MRI films, each of which must be accompanied with a report, or a medical oncologist’s summary and recommendations of your situation now, or office notes from your local breast healthcare practitioner’s office such as a breast surgeon, nurse practitioner, general practitioner, family medicine doctor, etc., and we will review those in advance.  The review conducted of these will be by either a breast surgeon or medical oncologist, depending on your particular situation.  There will not be subspecialty reviews in this level of service by either a pathologist or radiologist.  The response to your case will be comprehensive and personal and the talk that is done will be about you and your situation.

Premium Service

Urgent Requests

Finally, there may be conditions which you feel are of an urgent nature and you wish to have BREAST TALK within a 24 hour period of engaging us.  We are also able to meet your needs in that regard and will schedule that for you within the 24 hour timeframe.

Credentials

The credentials of the people that you are speaking with will be Paula Greene, A.R.N.P. or Robert A. Gardner, M.D., Fellow American College of Surgeons and those credentials are posted and, if you wish, be verified through the medical staff office of any of three hospitals that we are on staff at here in Palm Beach County.  The hospital names and numbers are included in our credentialing resume.  If a healthcare expert other than either of the two above is assigned to your situation and will be calling you, you will be given that person’s credentials in advance for you to review.  During the course of our conversation, you may take as many notes as you wish and we will repeat anything that we say as many times as you need us to until you are comfortable with an understanding of the information under discussion.  We will not be providing written reports of our telephone conversation.


Visit With Us

Finally, BREAST TALK may lead you to the conclusion that you would like to visit with us directly. In that case, arrangements can be made by calling our private offices at (561) 881-9100 and discussing your wishes with any person who answers our telephone.

You can fly to Palm Beach International Airport located 15 minutes from our offices and have a consultation the day of or the day after your arrival and we will assist you with that.  We will also make available services of our Center for Breast Care, our hospital, our pathology laboratories, and our imaging specialists to meet the needs that you may have in a time effective way. 

This new model of allowing you to receive information through BREAST TALK is not a substitute for a traditional history and physical examination, etc., in the form of a new patient consultation and/or a referral consultation.  It is not our intent to practice medicine over the telephone or internet, but rather to explain to you the information that is germane in your case and answer your questions in order to allow you to make choices regarding obtaining further care, etc., choices which can literally save your life or save your breasts.  By engaging us in BREAST TALK, it is specifically understood that we are not taking over your care and you are not engaging us for care.  That is a completely different issue and is outside of the scope of BREAST TALK.  It is an opportunity for you to have a prolonged discussion with a breast expert to give you the guidance you may wish to have and may, in fact, be in great need of.  By engaging in BREAST TALK, you are exercising your rights under the United States Bill of Rights to engage in freedom of speech and we are likewise utilizing those same rights to have a dialogue with you.  We will not be violating your rights to privacy under the various protection guidelines already assured.  You allow us to assign your situation to one of the other experts for BREAST TALK who are part of our BREAST TALK panel.
